The Inclusion Team use community mapping as a tool to encourage greater engagement of the person being supported in the community in which they live. The map is focused on relationships within the community and any activities, events or places where interaction and relationship building may happen.

The community map is also a way of the support team documenting the names of people, places the person supported likes to go and the times of events and where they happen. The community map is also a prompt to explore new possibilities within the neighbourhood. From our learning a person being support is more likely to make connections with people when they use local services and engage more in their own neighbourhood rather than travelling elsewhere to access services.
