Development Plan
identifies organisational goals for the coming year, identifying those which need to be carried into TAPs.
MOVERS
identifies individual goals for staff, the monthly supervision process, and individual training needs for the year ahead.
Training Plan
consolidates individual training needs to identify overall training needed for the year ahead.
Guidelines for Support
describes the main information for teams about how to support each person.
Risk Assessment
identifies risks in supporting people and ways to minimise / overcome these. Consists of a general risk assessment for life in the community, and specific risk assessments for new activities or changes to normal routines.
Top Tips
basic practical guidance as to what to do to successfully support the person.
Recording Systems
details of how and why certain information needs to be recorded – scatter plots, incident forms etc.
Health Action Plan
a summary of responses to people’s physical health needs including medical checks, dentists etc.
